Apply for Services

Applying for services at CVI is your first step towards independence. For your convenience, you can initiate the process by filling out the application and medical release form.

To complete the application via telephone or to find out if you are eligible for Medicare coverage, contact an intake counselor in the Client Services Department at 404-875-9011.

CVI OPEN HOUSE

If you or someone you know is visually impaired or blind, we welcome your attendance at our monthly Open House, held on the 4th Thursday of the month from 1:00 - 2:00 p.m. at the Center for the Visually Impaired. We will provide an overview of all of the adult services offered at the Center and explain the application process. Tours will be offered as needed.
